Please fill out the following form then print out a copy and send it to Plumas Lake Moms, PO Box 5393, Marysville, CA 95901-8531 with your membership fees. Please do not send cash, make checks payable to: PLUMAS LAKE MOMS.
Annual membership fees are $20.00 and are due January 15th. New members pay a prorated amount at the time they join:

I am aware that participating in the Plumas Lake Moms Group involves activities that may carry risk and I agree to accept any and all risks of injury to my child(ren) and/or myself that may result from our participation. The Plumas Lake Moms Group will not accept responsibility for any injury caused by participation in any event, including but not limited to: playgroups, park playdates, and field trips. Each member is responsible for her own child(ren) at any event and no other member will be held responsible for any injury that happens while at a member’s house or property.

The undersigned being the lawful parent(s) and/or guardian(s) of the above child (the “Child”), hereby consents to the participation by the Child in Plumas Lake Moms Group Activities conducted by Plumas Lake Moms Group and its members.

The undersigned assumes all risk of injury or harm to the Child associated with participation in Plumas Lake Moms Group activities and agrees to release, indemnify, defend and forever discharge the Plumas Lake Moms Group and its members (collectively the “Organizer”) of and from all liability claims, demands, damages, costs, expenses, actions, and cause of action (collectively the “Claims”) in respect of death, injury, loss or damage to the Child or by the Child, howsoever caused, arising or to arise by reasons of or during the Child’s participation in the activities.

I have carefully read this agreement and release of liability and fully understand its contents. By signing this agreement and release, I agree to all of the above.
